1. Foreign passport
- All pages with annotations.
- If you have two passports – all pages with annotations from both passports.
- If a Ukrainian citizen has no travel document or it is empty – PESEL in UKR.
2. Residence card + decision
- All pages of the card and the decision.
- If you are waiting for your residence card – confirmation of application submission. This may be, for example, a stamp in your passport, a certificate of application status from the Voivodeship Office, or confirmation from the post office.
3. Valid student ID
- Student ID card
- Certificate of study or a diploma of completion of full-time studies (from Poland or the EU).
4. Valid school ID
- School ID card
- Certificate from school or diploma of completion of post-gymnasium schools (only Polish schools count).
